A completed PADI Medical statement is required at the start of every training course, or once a year in respect of activities or organised dives.
If you have a condition requiring certification from your GP Specialist you can use the same signed form, providing there has been no change in your condition since it was originally signed by the doctor.
Remember please answer all the questions honestly, this isn't about diving its about your and our safety.
This form should be used if you are taking Advanced Open Water or any of the speciality courses and is designed to be valid for a period of one year for continuing education & courses conducted at the same dive centre or resort.
Teaching children comes with extra responsibility not only for us but also for the parent / guardian. PADI have produced a presentation to explain this in simple terms, your instructor will still undertake the presentation when you meet face to face. Following the presentation the responsible adult will be asked to sign the youth diving statement in conformation that the presentation has been given and understood.
In the event of an emergency we need your consent at provide "First Aid" to your child. This will only be undertake by suitably qualified staff, however this is required even for something as simple as a plaster.
This form should be used if are renting equipment from us. Student divers must also complete appropriate forms required for training (release/statement of risks, medical form, safe dive practices statement).
